changes toolbar buttons (not tb), options icons, add-on icons (uses 16px/32px icons)
tested on winxp ff1.5-3.5/a bunch of themes and tb1.5-3.0/default theme

Instructions:
download/rename one of the icon sets below to icons16.png/icons32.png and put the pngs in Program Files\Mozilla Firefox\icons (or Thunderbird)
you may need to open a new window or restart firefox after you apply the style

download all of them

for Firefox 3 users:
to remove the drop menu button. just below the @namespace line add
#back-forward-dropmarker{display:none!important}
